1. Completed Loan Application.
2. Driver’s license (a copy is required if you are not able to produce the original in person when returning this application).
3. Copy of Articles or Certificate of Organization.
4. Copy of Bylaws or Operating Agreement.
5. SBA Form 1919 for any owners 20% or more.
6. 4506-T form completed.
